09-10 Registration-RETURNING Student
The registration process for District 96 families who have students returning for the 2009-2010 school year has changed. Registration materials will not be mailed to your home in July. This year registration information and forms are available on the District 96 website.
You have the option to print the forms and fill them out or you can use the fillable forms and complete them on your computer. This is not an on-line registration process. You are required to send the completed forms and payment for school fees to your school. At this time we are unable to accept an electronic copy, as we are required by law to have a parent or legal guardian’s signature.
IMPORTANT DEADLINES
To ensure your child will be included in the class assignment, registration forms and fees are due by Wednesday, June 10, 2009. Registration forms and fees received after June 10th will be processed during the week of August 10, 2009 when school office re-open after summer break.
Return the completed forms and fee payment to the school your child will be attending in 2009-2010.
Please write 09-10 Registration on the envelope.
Registration forms not received by August 10th will be considered late registration. Late registrations will only be accepted on Friday, August 14, 2009 during normal school hours.

SCHOOL FEES (REQUIRED)
Please make check payable to “Riverside School District 96″. If you have children at multiple schools, a separate check is required for each school they will be attending.

STUDENT HEALTH EXAMINATIONS (IF APPLICABLE)
The following health examinations are required as indicated below. Completed forms should be returned no later than Friday, September 4, 2009.
