10-11 Registration – NEW Student
Registration for the 2010-11 school year for students new to District 96 will begin on Monday, May 3, 2010 and continue through Friday, June 4, 2010 during regular school hours. The parent or legal guardian of a new student to District 96 is required to register their child in person at the school office. If you are unable to register your child during the dates listed above you will need to register your child on Friday, August 13, 2010 between 9 a.m. and 3 p.m.
Registering your child May 3, 2010 through June 4, 2010 guarantees you are registered in District 96 but does not guarantee placement at your home school. School and class placement will be done the week of August 16, 2010 once all returning students have submitted their registration forms.
You must provide the following completed forms, supporting documents and fees, in person, at the time of registration for the registration to be considered a complete registration.
You have the option to use the Fillable forms and complete the forms on your computer or you can print the Print & Fill forms and write the information in. This is not an on-line registration process.

CERTIFIED BIRTH CERTIFICATE (REQUIRED)
An original certified birth certificate is required for each child entering District 96 for the first time. Copies will not be accepted.
PROOF OF RESIDENCE (REQUIRED)
A student must be a valid resident of District 96 and the parent or legal guardian must provide required proof of residency before the student will be enrolled. Residency is defined as the district wherein the parent or legal guardian resides.

You are required to provide at least five (5) original and unaltered documents from the following list that displays a current address within the District 96 boundary. Copies are not accepted.
Provide the following original documents:
A. Drivers License or State Identification Card – must be current and display an in-district address
B. Homeowner title, mortgage statement or apartment lease with the landlord’s contact information
C. Plus, at least three (3) of the following recent items:
Utility Bill: phone (landline/cell), electric, gas, water, Internet or cable
auto insurance, homeowners’ insurance credit card statement, bank statement, paycheck stub or Medicaid statement

Due to recent changes in the way the Illinois State Board of Education collects data regarding race and ethnicity, ALL, new students must complete the ISBE Department of Education Race & Ethnicity Data Collection Form.

STUDENT HEALTH EXAMINATIONS (REQUIRED)
The following health examinations are required as indicated below. Completed forms should be returned no later than Friday, September 4, 2010.

DISTRICT 96 HEALTH FORMS
Please complete the applicable form if your child has any of the following medical conditions:
- Asthma that requires the use of an inhaler at school.
- Food Allergies that requires an Epi-pen and/or Benadryl to be kept at school.
- A medical condition that requires your child to receive medication at school.
Please have your physician complete the appropriate health forms and return them to your child’s school no later than the first week of school. Please make sure to complete and sign the parent section.
Page 13 of the Parent Handbook explains medication usage at school. If you have any questions, please contact your school nurse.
SCHOOL FEES (REQUIRED)
There will be NO increase in the base registration fees for the 2010-11 school year.
This year, the registration fees and miscellaneous fees have been combined into one fee for 6th, 7th and 8th grades as indicated below.
Grade 6: Fee includes the base registration fee, Encore class fee, workbook, lock, heart rate strap and assignment book fees.
Grade 7: Fee includes the base registration fee, Encore class fee, workbook and assignment book fees.
Grade 8: Fee includes the base registration fee, Encore class fee, workbook, assignment book fees and graduation gown fee.

Please make check payable to “Riverside School District 96″. If you have children at multiple schools, a separate check is required for each school they will be attending.
TRANSFER OF SCHOOL RECORDS
Parents or legal guardians should request the transfer of school records if your child has attended school in another district.
