11-12 Registration – NEW Students
The parent or legal guardian of new students to District 96 must register their child in person at the District Office.
The District Office is located at 2400 S. 10th Avenue in North Riverside. The entrance is at the south end of Mater Christi Catholic School.
Office hours are as follows:
Monday – Friday 9 a.m. – 4 p.m.
Registering guarantees you are registered in District 96 but does not guarantee placement at your home school. School and class placement will be done the week of August 22, 2011 once all returning students have submitted their registration paperwork.

You must provide the following completed forms, supporting documentation and fees in person.
You can complete the forms on your computer or you can print them and write the information in.
CERTIFIED BIRTH CERTIFICATE (REQUIRED)
An original certified birth certificate is required for each child entering District 96 for the first time. Hospital issued birth certificates are not acceptable. Copies will not be accepted.
REGISTRATION FORM (REQUIRED)
A registration form is required for every student.

PROOF OF RESIDENCE (REQUIRED)
The student must be a valid resident of District 96 and the parent or legal guardian must provide required proof of residency before the student will be enrolled. Residency is defined as the district wherein the parent or legal guardian resides.

You are required to provide at least five (5) original and unaltered documents from the following list that displays a current address within the District 96 boundary. Copies are not accepted.
Provide the following original documents:
A. Drivers License or State Identification Card – must be current and display an in-district address
B. Homeowner title, mortgage statement or apartment lease with the landlord’s contact information
C. Plus, at least three (3) of the following recent items:
Utility Bill: phone (landline/cell), electric, gas, water, Internet or cable
auto insurance, homeowners’ insurance credit card statement, bank statement, paycheck stub or Medicaid statement

STUDENT HEALTH EXAMINATIONS (REQUIRED)
The following examinations are required for all new students to District 96. Completed forms should be returned no later than Wednesday, August 24, 2011.

Your child will not be allowed to attend school if the above health forms are not turned in by August 24, 2011.
DISTRICT 96 HEALTH FORMS
Please complete the applicable form if your child has any of the following medical conditions:
- Asthma that requires the use of an inhaler at school.
- Food Allergies that requires an Epi-pen and/or Benadryl to be kept at school.
- A medical condition that requires your child to receive medication at school.

Please have your physician complete the appropriate health forms and return them to your child’s school no later than the first week of school. Please make sure to complete and sign the parent section.
Page 13 of the Parent Handbook explains medication usage at school. If you have any questions, please contact your school nurse.
SCHOOL FEES (REQUIRED)
The school fees listed below are pending Board approval. If there is a change in the fee structure you will be notified.
Grade 6: Fee includes the base registration, Encore class, workbook, lock, heart rate monitor strap and assignment notebook.
Grade 7: Fee includes the base registration, Encore class, workbook and assignment notebook.
Grade 8: Fee includes the base registration, Encore class, workbook, assignment notebook and graduation gown rental.
Please make your check payable to “Riverside School District 96.” If you have children at multiple schools, a separate check is required for each school they will be attending.

The following items need to be purchased for new 6th, 7th and 8th graders only. These items are available for purchase at the Hauser Junior High School Office once your child’s registration has been approved.

IMPORTANT: Locks must be school issued by the Hauser office.
TRANSFER OF SCHOOL RECORDS
Parents or legal guardians should request the transfer of school records if your child has attended school in another district.
