If your child is participating in Conference Athletics the following forms are required.

Once completed please return to the School Health Office.

For your child’s safety they will not be able to tryout for any sport unless the Sports Participation form and the Sports Physical Form (7th-8th), IL Heath Examination (6th) are completed and received.
